LIFE Geisinger is hiring an Intake Coordinator to attract, educate and enroll new participants to the program. Our enrollment department interacts with all members of the LIFE Geisinger Team and can be creative with attracting new members. Join our team today.
Responsible for all aspects of the enrollment process for the LIFE Geisinger program. Assists Marketing Staff with outreach activities to increase referrals and grow census.
Communicate details of the program to potential enrollees and families. Evaluates potential enrollees to determine needs and eligibility for the program. Schedules and completes home visits for all potential enrollees. Conduct tours of the facility to potential enrollees and families to sell the program. Completes intake assessment and all relevant paperwork for the enrollment process. Initiates and follows through with the completion of medical assistance application to ensure payment. Communicates with Interdisciplinary Team details regarding the enrollee, dates and times of trial visits and plan for enrollment signing. Establishes and maintains effective working relationships with county agencies, referral sources, community groups and all other stakeholders. Communicates with referral source regarding outcome of the referral in the enrollment process. Coordinates support services and other appropriate referrals for those not enrolled in the program. Promotes the program to increase referrals and grow census.

High School Diploma or Equivalent -Required
· Minimum of 6 years relevant experience -Required *
· Minimum 1 year of experience working with the elderly or frail – Required
· Minimum 1 year of Sales experience-Preferred
· Bachelor’s degree in marketing, social work, communications or psychology is preferred
· Experience working in the community is critical
· *Relevant experience may be a combination of related work experience and degree obtained (Associate’s Degree = 2 years; Bachelor’s Degree = 4 years).
Extensive local travel in 4 county region is required. Mileage reimbursement is available. Work is typically performed in an office environment, community or participant’s home.
