The Cancer Patient Navigator offers individualized assistance to patients, families, and caregivers throughout the cancer care continuum. The Navigator works to overcome health care system barriers; facilitates timely access to medical and psychosocial care; provides guidance and assistance with financial issues including insurance, payment systems, and other financial support; and offers education to assure patients and families have the resources, information, knowledge, and support they need through all phases of the cancer experience. This requires case finding, patient/family needs assessment, identifying and mobilizing resources, follow up, and communication with the patient/family and members of the health care team. Functions as a member of the health care team working toward optimum patient outcomes.
DUTIES AND RESPONSIBILITIES:
Breast Center - Works closely with Breast Center Navigator, the breast imaging staff and physicians, and surgeons to provide patient education and navigation.
Multi-Disciplinary Conferences - Coordinates and hosts the cancer conferences to include case finding, physician notification, room set-up, prepare case summaries, attend and take notes, physician follow up, track attendance and relevant data, and maintain physician CME credits.
Navigates care for cancer patients and their family.
Schedules appointments for physician visits and treatments, ancillary services such as lab, pharmacy, diagnostic and screening imaging, genetic counseling, social services, nutritionists, dieticians, psychologists, psychiatrists, chaplain, cosmetology, image center, and other important care or services need.
Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies.
Serve as an essential link between patients and all other care providers.
Facilitate appointments for consults and support services within established service standards, including social workers, dietitians, physician, etc.
Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
Develop patient education programs and tools.
Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources.
Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes.
Deliver quarterly report card to Virtua committees and other groups which demonstrate outcomes and performance improvement activities.
Adhere to established productivity benchmarks.
Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff, etc.).
Performs other duties as assigned.
Maintains records pertinent to personnel and operation of the unit. Prepares reports, i.e., utilization, performance improvement, outcome data.
Participates in patient care conference.

Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict.
Adheres to dress code.
Completes annual educational requirements.
Maintains regulatory requirements.

Attends department staff meetings as required within the department.

Actively participates in performance improvement and continuous quality improvement (CQI) activities.
Coordinates efforts in meeting regulatory compliance, federal, state and local regulations and standards.

Complies with Health and Safety Standards and Guidelines.
Licensed in the State of Montana as a registered nurse.
Three or more years of oncology experience preferred.
Case management or discharge planning experience preferred.
Requires problem solving, decision making, and critical thinking.
Requires excellent leadership, organizational, written, and verbal communication and excellent interpersonal skills.
Must be able to work in a self-directed environment, with an ability to work with and lead teams.
Strong presentation skills.
Ability to implement professional and community based education programs.
Strong computer skills, Microsoft Office required.
